This year we will be using an alternative method to furnish your Form 1095-C, in accordance with the IRS instructions for Forms 1095-C.
Instead of mailing you a paper copy of Form 1095-C, we will make your Form 1095-C available electronically through a secure online portal. Form 1095-C provides important information about the health insurance coverage offered to you, as required under the Affordable Care Act, and may be useful for your tax records. We will let you know when Form 1095-C is available and provide instructions for accessing it. You will have the option to request a paper copy at no charge, which must be fulfilled within 30 days of your request.
